This is often a permissions issue.  Make sure that apache has write permission
to both the course directory and also the webwork2/htdocs/tmp directory.

If this doesn't solve the problem, can you post the results of the following:
when you click on the (missing) image, it should try to open it in a new
window.  What message do you get in that new window?
