- I would look for problems which use the labeled_ans_rule method.

trigger the issue but that is my first guess. 

- Finding the problem/problems causing the issue is going to be a matter of
looking at the PG code and/or removing them from the set. 

- Having the assignment type be Gateway/Quiz could have an effect.  If its the
issue I'm thinking of then changing it to "Homework" would fix everything. 

- The "student progress" errors are probably related to this.
