New problems not being saved to local problem set (HWKSet)

We are having an issue saving new problems to the local problem set. It says that it has saved it, but it actually has not. I am not sure where to look. Any help available.

It is running on an ubuntu 14.04 system.

Last night and this morning I was unable to add problems to a problem set in the END1021F_2015 course. The problem set is Area and I was selecting problems from Rochester/setIntegrals19Area from the OPL directory. The Green highlighted bar appears saying that 1 problem has been added to the set but the set remains empty. This didn’t happen when I filled my other problem sets.
