Has anyone managed to write a PG file that conducts a survey and/or the necessary things to gather the results from it? I remember there was a mention of something a while back, but I never managed to get the source code (or really see how to do it for that matter).

Take a look at the examples in setSampleQuestionnaires:

and see if they can be modified for your needs.

Both locations also contain scripts to compile the results. To see what our current survey looks like, login as a practice user to http://webwork.math.rochester.edu/mth161/ and look at problems 1 and 2 in set 13.
