You should hear back from MAA within 48 hours, usually less.
Why don't you submit the form again -- it may just have slipped by.

After I completed the form for the trial membership a page came up with the following message
The MAA Service Center will call you at the number provided in the request form to arrange payment. Please note that MAA will not be able to create your course until after the payment is received. Once the payment is processed, MAA will set up your course and email you the course URL and login information.
but that seems to be connected to the paid hosting membership requests. In particular there was no "number" in the request form.

yes, it is.
Just the acknowledgement page that comes up after free registration sounded to me to be an acknowledgement page for the paid subscription. A cosmetic issue.
